TRANSCRIPT REQUEST FORM

Students requesting high school transcripts for out of state schools will need to complete the form below. There is a 2.00 fee for every transcript.

It is recommended that students use GaFutures for in state transcripts. If students prefer to have the transcript mailed, there is a 2.00 fee and the form below should be completed. Once the transcript is requested, Ms. Bradshaw will release it to the college.

There is no charge for students requesting transcripts for Scholarships.

*Please remember that official test scores must be sent by the student. The scores must be sent by the testing sites.
