2. Complete the Online Drug Program Screening Consent Form.
3. Pay $80.00 on MyPaymentsPlus.com.
4. Upload your confirmation of payment (screenshot or photo) to the google form.
5. Upload a picture of your license to the google form.
6. Upload a picture of your insurance card to the google form.
